This template has to be placed on the bottom of a disambiguations page. It will automatically add the Category:Disambiguation pages to the page.
A disambiguation page is made when multiple pages should have the exact same name. Instead of placing one under the original name and renaming the other, you place a disambiguation page on the original title with links to both other pages.
